REGISTRATION OF A DEATH

 

 

To register a death you will need the following:

 

The Death Certificate from the Doctor

 

Also if possible

 

The National Heath Service Medical Card, N.H.S.Number or Identity Card.

The Birth Certificate.

Any Pension Book which is in use.

 

In normal circumstances you will need a Medical Certificate from the Doctor who attended the deceased (this will be addressed to the Registrar) together with a Formal Notice that states the Medical Certificate has been signed. Please see ‘Registration Information List’ to help you provide the Registrar with the information required.

 

The Registrar will issue a Certificate for burial or cremation (The Green Form) which should be delivered to us as soon as possible. A Certificate of Registration of Death (The White Form) will also be issued to you, together with the relevant leaflets, for Social Security purposes.

 

At this stage it may be advisable for you to ask the Registrar for copies of the Death Certificate which may be needed for legal and financial matters concerning the deceased's estate. There will be a small charge for these.

 

Registration Information  List

You will be expected to supply the following information:

1.     Date and place of death

2.     Name and surname of deceased

3.     Sex

4.     Maiden name of woman who has married

5.     Date and place of birth of deceased

6.     Occupation and usual address

7.     Your full name and relationship to deceased

 

In addition, for the Population (statistics) Acts, the following information:     

1. At the date of death was the deceased single, married, widowed or divorced.

2. If the deceased was married at the date of death, state the date of birth of the deceased's widow or widower.

REGISTRATION OF DEATHS IN THE REGISTRATION DISTRICT OF TROWBRIDGE

 

The Office opening hours for the registration of Deaths in the Registration District of Trowbridge are as follows:-

 

Appointments will be 20 minutes apart and the first will be 9.20a.m.and the last 11.20a.m. Time towards the end of the morning session will be saved for emergency and urgent callers. The afternoon appointments will be from 2.20p.m. to 3.10p.m.
